The accident happened around 6:40 a.m. on the eastbound side of Interstate 74, near milepost 211.5. A Chrysler minivan driven by a 28-year-old man was heading west on the westbound side of the freeway, carrying seven passengers. For an unknown reason, the minivan struck a concrete bridge support, overturned, and stopped in the eastbound lanes. The minivan was then struck by an eastbound car and up to nine other vehicles at the scene became involved either in the original accident, or chain reaction crashes.

One passenger inside the minivan was tragically pronounced dead at the scene, and was identified as 29 year old David A Miller of Dundee, Ohio. Three other people were rushed to hospitals after the accident, and the exact condition of all those involved is unknown at this time. The investigation is ongoing.
