The deadly collision occurred at approximately 8:01 a.m. when a minivan and a silver full-sized truck, occupied by Ortega, collided. One witness stated that the truck was trying to avoid someone, while traveling at a high rate of speed, crash into the minivan and rolled over several times. A total of 7 occupants were inside the truck.

Ortega was pronounced dead at the scene. Another teenager, later identified as 14-year-old Simon Sotelo, was transported to Highland Hospital where he later succumbed to his injuries. The five other victims of the collision were transported to the hospital for treatment.

Good Samaritans nearby ran over to assist the victims of the collision. The woman in the minivan was transported to the hospital for non-life threatening injuries. An investigation is ongoing to determine the cause of the accident.
